Assessing Community Healthcare Needs

Assessing the health requirements of the community is necessary to validate that proper health and safety are provided to the community which are effective in fulfilling the patient’s needs regarding care coordination and health facilities (Ravaghi et al., 2023). The assessment of the health needs of Jordan a small town in the Minnesota countryside helps in the identification of the general needs along with the safety concerns and limitations of the Jordan. It will identify the environmental factors and the social determinants affecting the health of the patients and summarize the findings of the environmental analysis and a virtual windshield survey for effective data collection.

Executive Summary

General Condition and Needs of a Community

Jordan is a small town but it is an effectively maintained community located in Minnesota having health care services obtained through St.Francis Health Services. There are a lot of parks including Lagoon Park in Jordan which are developed for the fun activities of the people living there however, few land is covered for business development. This community has limited transportation and bad-conditioned sidewalks in rural areas thus creating access issues for the people (Thondoo et al., 2020). The town lacks most of the dietary foods and noticeable fast food places which creates health concerns for the individuals. The people of Jordan are facing an increase in chronic diseases along with limited resources for the management of mental health.

Applicable Safety Concerns

The sidewalks in the rural parts of Jordan are dangerous thus creating health and life risks for the people (Khader et al., 2023). The handling and management of emergencies possessing huge damages can not be treated at St.Francis Health Services due to limited health resources. The limitations of transportation are the major factor in the access to health facilities on time thus creating health issues.

Limitations of the Survey

The survey did not provide complete details about the real health needs of the people as it lacked direct contact and the identified issues are limited due to the time constraints. However, some health issues related to the environment and other diseases are still unidentified.

Environmental Factors Affecting Population Health

The situation of homes in Jordan is well maintained but in rural areas old homes are present which creates health issues due to poor house conditions including mold, fungus, and so on. Limited transportation also creates health issues, the lack of rural places having fun parks and insufficient access to specialized care and health safety at the clinic due to limited resources (Varela et al., 2019). However, the lack of education and support from community networks is also insufficient due to extinct resources. Low income and unemployment along with limited access to dietary food are also concerning factors (National Institute on Minority Health and Health Disparities, 2024). The increasing number of chronic diseases and limited mental health services are also important environmental factors impacting the health of the population.

The key health issues include ineffective management of chronic disease and mental issues of the people leading to the exacerbation of the situation (Wan et al., 2021). The limited transportation facilities and insufficient knowledge and education of the patients regarding prevailing diseases and their management are the key health problems.

Opportunities

The coordination with Allina Health will be beneficial for the healthcare providers of the Jordan community to improve patient outcomes. Providing education and improving the management of mental health issues are the most important opportunities that need to be availed to ensure access to healthcare for all people.

Threats

The increase in the rate of chronic diseases and the limited healthcare functionalities of the clinical facilities are the major threats to the prevention of health damage leading to a negative impact on the clinical facility (Holman, 2020). The financial situation of the people and limited access to healthcare settings also pose significant threats for the people.

Social Determinants of Health

​​The economic situation of Jordan normally shows stability however some old areas and towns are facing low-income issues having limited access to health services. The financial situation depends on agriculture, small businesses, and other small services. The increased fast food chains and the presence of deserted lands in rural areas also cause hindrances in the way of acquiring quality health services (Erpecum et al., 2022). Insufficient educational resources and limited access to health education are some of the major social determinants of health. The rural areas of Jordan due to limited access are often eliminated from the basic health services however support from community networks is effective in managing the health of the patients (Khader et al., 2023). The limited transportation and lack of access to health services are affecting patient satisfaction at a large scale.

Underlying Assumptions

The assumption is based on incomplete data about the income situation of the Jordan people and without any specific information it has been assumed that some areas of Jordan have no access to quality food. It has been assumed that all the population has equal education levels regarding technology and diseases and some areas have limited access to health services without any data.

Summarizing Environmental Analysis and Survey Findings

The identification of the health needs of the Jordan community provides a deep insight into the environmental factors affecting the health of the population along with the conduction of a windshield survey that provides more knowledge regarding the health condition of the community and the factors that are necessary for the improvement of the health outcomes of the patients. Jordan has stable economic conditions however some differences occur in the financial situation of the people due to areas like low-income households in rural areas having limited access to health services. The educational availability is good in the town but some areas mostly rural have limited and insufficient access to educational resources thus becoming a major area of concern for the healthcare providers (Coombs et al., 2022). The town has strong support and bonds with the community networks regarding the health facilities however some groups feel left out due to different social factors. The health resources are also limited for the rural people along with increased health damaging risks for the people living in rural and old areas.

The assessment of the health needs of the Jordan community located in the Minnesota countryside helps in the identification of the threats and opportunities related to the health conditions of the people living in that town. Different environmental factors identification helps the support groups and networks to build strategies that are significantly effective for both the patients and the healthcare providers thus ensuring the availability of health for all the members (Kwame & Petrucka, 2021). The increase in the education and knowledge of the population and removing of discrimination based on rural living of the people can be effectively used to improve the health outcomes of the people and increase awareness regarding the preventive measures for the improvement of the level of patient satisfaction.
