Planning for Community and Organizational Change
The identification of the health needs of the community is necessary to validate the provision of care and safety to the patients according to the needs of the community (Ernstmeyer & Christman, 2022). Determining health issues will help the community support network and other stakeholders to introduce practice change according to the requirements of the community members. The change of practice is effective in implementing strategies that efficiently address the health issues of the community and improve the health outcomes of the patients. The determination of the health needs of the Jordan located in the Minnesota community helps in the development of the change in practice plan needed to ensure the efficiency of the new practices in the community healthcare service for the betterment of the patient response. Proper planning and effective implementation of the change will generate results that are important for the patients of the community. Moreover, the process of implementation will help in the identification of the barriers affecting the change and the approaches that can be used to improve those barriers and convert them into opportunities (Alshwayat et al., 2022). Developing the understanding of the stakeholders to effectively evaluate the proposed changes and identify the budget needed for the change to ensure the sustainability of the change.
Implications of Proposed Healthcare System Change For Jordan
Jordan is a small town in which the rural areas are mostly deprived of most of the facilities including transportation which results in the delay in the treatment needed to provide to the patient for the improvement in the health situation. This transportation problem can be resolved using telehealth service which provides the functionality of maintaining the health situation of the patient at any place without having the patient move physically (Haleem et al., 2021). It will also save the cost for patients having financial problems who are unable to pay for daily hospital visits. The conduction of educational programs is necessary for the improvement of the education of the healthcare providers regarding the prevention of health diseases and effectively managing their health through removing the knowledge barrier and increasing self-management. Through the use of mobile applications, healthcare providers will be able to detect health problems early and provide timely treatment to patients.
Implications of Proposed Changes
The integration of healthcare services using telehealth will resolve the issue of ineffective health delivery due to transportation issues, especially in rural areas. The reduction in the disease will occur due to the increase in the self-management of the patients and the community will be more aware of the disease prevention strategies (Maina et al., 2023). The overall health needs of the community can be fulfilled with the implementation of mobile applications.

The reduction in the prevalence of chronic disease is one of the major outcomes of the proposed change in the Jordan clinical facility. The high cost of telehealth devices creates financial problems for the community of St. Francis Health Services. The training schedules other than work will create the fear of adopting new technology and managing training sessions. The alignment of the technological integration with the rules and guidelines provided by the regulatory bodies is necessary and acts as a resistance as it needs effective practice.
Strategies for Changing Barriers into Opportunities
The integration of technological advancements in the clinical setting of Jordan can ensure the conversion of the barriers into the opportunities necessary for the improvement in the health response of the patients and resolving identified health issues (Berardi et al., 2024). Involving the people in the educational programs to increase knowledge through the conduction of workshops, training programs, and so on. Considering the cultural preferences of the people and integrating the cultural requirements in the change plan is the major factor for the resolution of the barriers and increasing the opportunities. Removing financial barriers is necessary for the implementation of the technology which is effective for the access of health services to the rural areas. Considering the concerns of the staff members regarding the implementation of technology is necessary to ensure that the change is effective. Deliver proper information regarding the roles and responsibilities of the stakeholders and ensure that they all participate in the board meetings. Provide clear and accurate data to the stakeholders regarding the outcomes of the patient using telehealth services and present the effectiveness of the cost management using proposed changes. Providing all the necessary information to the stakeholders and the decision makers so the gaps in practice can be effectively determined and improved before the implementation of the changes to ensure the cost saving and effectiveness of the change. Answer all the related questions of the decision-makers through the surveys and questionnaires and conduct the assessment of the impact of the changes on the clinical facility and the stakeholders. Conduct surveys in different situations using different technologies to identify the effectiveness of the change. Proper evaluation surveys should be developed to effectively evaluate the outcomes of the changes and frequent feedback should be taken from the patients and the stakeholders to evaluate the quality metrics (Schöpf et al., 2019). It will increase the access to healthcare facilities for rural people and enhance their level of satisfaction and well-being with the health services provided to them. The basic purpose of the change is to reduce the rate of hospital readmissions by providing quality health services to patients and increasing access to health services for the population having transportation issues and other potential barriers (Dhaliwal & Dang, 2024). The increasing rate of chronic diseases is also one of the major factors in the implementation of the change which includes the integration of telehealth services, mobile applications, and staff training along with patient education. The different costs include the cost of technology intervention, training programs, administrative expenses, and the cost of technology installation. The clear administration and availability of these cost factors are necessary for the success of the changes implemented.

The determination of the potential barriers is necessary to ensure that all the areas of concern are identified and significantly improved using change management strategies. The strategies are implemented for the increasing engagement of the stakeholders in the change implementation process and a proper budget was associated with the integration of the technology in the Jordan healthcare community.
