Advocating for Lasting Change

Slide 2: Hello, I am —. The topic of today’s presentation is implementing the changes that will last for a long and improve the healthcare situation of the people of the Jordan community located in Minnesota. The identification of the health needs of the population of Jordan highlights the importance of the practice changes in the clinical facility for the improvement in the response of the people regarding the health services provided to them. The implementation of the practice changes can be effective if they are properly evaluated by the engagement of all the stakeholders and the healthcare providers (Petkovic et al., 2020). To make the changes last for a long it is necessary to provide financial support for the changes along with the development of an effective policy. Proper evidence needs to be integrated to promote changes in the clinical facility. The estimation of the budget and the plan for the implementation of evidence-based changes is necessary to ensure that the changes applied are sustainable and proficient in increasing the positive outcomes of the patients. Identifying the future sustainability of the change through the evaluation of its effectiveness and involving the interest of the stakeholders will improve the outcomes of the Jordan community.

Policy and Financial Support for Proposed Changes

Slide 3: Policy is necessary to be implemented to ensure that the change is sustainable and effective for the community and the healthcare providers. A policy provides a structured approach for the implementation of the changes and the effective criteria for the unified evaluation of the outcomes of the changes (Cassidy et al., 2021). The evaluation is necessary to ensure that the implemented changes help develop policies that consider the health requirements of the patients and improve the health services for the vulnerable and affected population of Jordan. The implementation of telehealth services requires a framework that will comply with the HIPAA rules and regulations for the safety and security of the patients. Proper licensing is required for healthcare providers to ensure the telehealth integration in the clinical facility by integrating the policy for equality in the access of service to the patients. Financial support is one of the major factors in ensuring the effectiveness of the changes applied for addressing the health needs of the patients and improving health outcomes.

Rationale for Policy and Financial Support

Slide 4: The most important rationale for the implementation of telehealth services in the clinical facility is the improvement of the health of the patients by providing timely and effective care coordination and self-management plans to the patients. Resolving the problem of limited access to healthcare services for rural people is necessary to ensure that equal health facilities are provided to all patients and barriers are removed for effective outcomes for the patients (Coombs et al., 2022). The assumption regarding the sustainability and success of the change is based on the idea that policy and financial support are effectively provided and implemented for the improvement in identifying and resolving the health issues of the people of Jordan.

Evidence that Proposed Changes to the Healthcare System

Slide 5: The implementation of telehealth services in the clinical setting has shown significant importance by generating positive health outcomes and increasing the rate of patient satisfaction with the quality of treatment provided to them. Telehealth services have increased the well-being of patients by reducing the rate of hospital readmissions and improving the self-management of patients regarding chronic health diseases including hypertension, diabetes, obesity, and so on (Drake et al., 2022). Through self-management knowledge and education, patients can improve the outcomes of health conditions by involving themselves in the monitoring and evaluation of the daily routine factors necessary for the improvement of their health condition. In Jordan, the issue regarding ineffective healthcare in rural areas due to transportation problems and other discriminatory factors has seen a significant shift toward the improvement and access to health for all as a fundamental right of the people. The reduction in the rate of hospital readmissions and improvement in the health of people belonging to rural areas having reasonable health barriers highlights an effective decrease in the cost of treatment and financial stability of St. Francis. It also shows the engagement of the community in the support of telehealth services and providing quality education to the healthcare providers for the effective response of the patients.

Perceptive Analysis of the Evidence

Slide 6: The integration of telehealth services improves patient outcomes in St.Francis by providing early intervention of healthcare providers in case of deteriorating health conditions and increasing the effectiveness of the regular monitoring of the health situation of the patients. This technological intervention allows the continuous communication of between professionals and population of the Jordan for the improvement in their health outcomes. The facility of helathcare at home increases the satisfaction and comfort of patients living in isolated geographic locations and having limited transportation and financial resources (Lankin et al., 2023). The financial condition of St.Francis improves with the decrease in the rate of hospital readmissions and retest procedures. The involvement of the community in providing quality care for improves the overall environment of the community and increases patient participation in health improvement.

Estimation of Broad Budget

Slide 7: The budget is the most significant factor in the successful implementation of the telehealth services in St.Francis which needs to be effectively considered and evaluated for the sustainability of the change implemented. The estimated budget for the implementation of telehealth in St.Francis is $340,000 which is an overall estimation divided into different categories. The financial resources availability is necessary to ensure that the updated system has advanced features that can effectively improve the health outcomes of the patients by increasing the effectiveness of the health services provided to them.

Key Funding Requirements

Slide 8: The key funding areas for the implementation of telehealth in St.Francis include the cost of the telehealth managers for monitoring the implementation process, an information technology expert, and the devices needed for the telehealth installation. The education of the healthcare providers regarding the use of telehealth services is necessary to be considered as it will also cost a great amount in the annual budget for the implementation of the advanced changes in the existing health system of the clinical facility of St.Francis. The publications and marketing campaigns regarding the effectiveness of the health services also needed a huge budget.

A Plan for Leading Transformational Evidence-Based Change

Slide 9: The transformation of the existing healthcare system to advanced healthcare facilities is necessary to ensure the provision of care and safety to all and increase access to health services and care coordination for the rural areas of the Jordan community (Gizaw et al., 2022). The vision of the organization is to implement strategies that are effective in increasing the rate of patient satisfaction and well-being regarding the health services provided to them by improving the management of chronic diseases, increasing the knowledge and education of the patients, and reducing the rate of hospital readmissions by assuring the quality of care for the patients at the first step. All these outcomes are administered and proved through the implementation of telehealth services in the clinical facility and increased efficiency of evidence-based practices.

Strategy for Leading Change

Slide 10: The change can be effectively integrated using Kotter’s 8-step change model which includes the following steps;
  • Identify the health issues and health barriers faced by the rural population of the Jordan community located in Minnesota.
  • Develop a team of healthcare members including experts, specialists, nursing staff, and healthcare professionals for the change implementation.
  • Communicate about the intervention of telehealth to the stakeholders involved for their decision.
  • Provide training and information to the stakeholders regarding the effective use of telehealth in the clinical facility.
  • Develop milestones for the implementation of telehealth services and divide the process into smaller steps.
  • Evaluate outcomes and use them for further improvement in the implementation of the services.
  • Validate the effectiveness of telehealth in the clinical facility by evaluating the outcomes of the patients.

Assessing the Potential Future for Wellness

Slide 11: The implementation of the telehealth services in St.Francis ensures the effectiveness of access to health facilities for the patients of rural areas having transportation issues, low-income rates, and insufficient education about the technology and its application. The increase in the access to health services for patients improves the outcomes of the patients about the health services provided to them and maintains the positive environment of the community about the balance in health access. Telehealth will improve the patient’s health and reduce health disparities by providing an early response to the patients in case of deteriorating conditions and increasing the well-being of the patients by providing early intervention of the healthcare providers (Ezeamii et al., 2024). Moreover, it provides the facility of regular monitoring of the health situation without having to visit the hospital. This remote monitoring increases the effectiveness of the treatment and reduces the barrier of limited transportation and financial resources for daily hospital visits. A sustainable health environment will be created for effective patient-centered health treatment.

Role of Visionary Leaders

Slide 12: The role of leaders is to provide quality support to the healthcare providers and other staff members for the effective implementation of the change in the clinical facility and encourage the stakeholders to participate in the change integration process for the improvement of the health outcomes of the patients. Leaders can motivate healthcare providers the integrate technology into their daily practice to ensure that they are effective in managing the health situation of the patients especially those located in rural areas (Laukka et al., 2020). They can improve the cooperation between healthcare providers regarding the identification of practice gaps and improvement in the areas of concern.

Assumptions

Slide 13: The assumption is that patients will be able to effectively integrate telehealth technology into their treatment and acquire education about the implementation of telehealth services. Healthcare providesr will effectively arrange their schedules learn about telehealth services and include the healthcare practices in their performance for the improvement of the patient’s response. Financial and policy support will be evident in provdiing quliaty services to the patients.

Stakeholders’ Interest for Proposed Changes

Slide 14: The engagement of stakeholders in the implementation process of the health facilities at St.Francis is necessary to ensure that all the stakeholders are communicated about the need for the telehealth service and how these services can be integrated into the system for the improvement in the health responses of the patients. Communication and education must be provided to the stakeholders by clearly defining the health barriers faced by the rural population of Jordan and helping them analyze the situation through the use of evident data (Mseke et al., 2024). The patients and their families should be part of the educational programs about the implementation of telehealth services for their better understanding. Healthcare providers should be effective in integrating the changes in the clinical facility through the support of healthcare leaders. The policymakers must ensure that the change in practices is in alignment with HIPAA rules and regulations for the safety and security of patient information. The risks associated with the implementation of telehealth include insufficient knowledge of the patients especially older adults about the technology and the availability of the internet resources for the people.

NURS FPX 6218 Assessment 4 Conclusion

Slide 15: The change can be effective and sustainable if proper policy and financial resources are identified for the change implementation. Proper evidence of the changes and estimated budget will be discussed to ensure that the changes can effectively fulfill the health needs of the patients (Stadhouders et al., 2019). A proper plan for changing sustainability through the consideration of future impacts will improve the practices of healthcare providers and other stakeholders. The interest of stakeholders is necessary for the effective implementation of teh change in the clinical facility.
