Abstract

The emerging technologies in the healthcare facility make it the need of time to integrate advanced healthcare technologies into the system for better outcomes for the patient and improved healthcare results (Junaid et al., 2022). This rapid change in the healthcare industry introduces various new technologies in the clinical setting that are more effective and provide high-quality healthcare service to patients. The ability of nursing staff to integrate the technological system in the healthcare facility and improve the outcomes of the patients is necessary as it will play an effective role in the organization and the clinical practice. Nursing informatics holds great importance in improving the response of patients by providing early healthcare services to the deteriorating health situation of the patients thus improving the patient outcome and reducing the health disparities. Hypertension is a chronic disease that is increasing and older age people are more vulnerable to this disease. The proper hypertension management plan and tools need to be integrated into the healthcare facility for the positive response of the patients (Omboni, 2019). The best nursing informatics tool that can be used to keep the level of blood pressure monitored and prevent it from elevating to hypertension, is electronic health records (EHR). This system is used to keep the patient’s medical history including hospital visits, previous levels of blood pressure, and regular monitoring of the blood pressure for the effective intervention of the healthcare providers. The ADKAR  model will be used to effectively implement EHR in a clinical setting.

Introduction

​​The EHR systems are based on advanced technologies that are essential to effectively monitor the patient’s health condition and provide them with high-quality care coordination for better outcomes (Ali et al., 2023). The rate of hypertension is increasing in older people which highlights the need to effectively integrate strategies that are beneficial for the positive outcomes of the patients and the effectiveness of the healthcare organization in dealing with the hypertension disease. The chronic disease of hypertension can be effectively improved using nursing informatics for the timely intervention of the healthcare providers and the effective treatment of increasing blood pressure. The level of blood pressure lies between 130-139/8-89 mm Hg in the hypertension situation which needs to be effectively controlled and monitored for the prevention of major health problems leading to increasing the rate of health disparities and hospital readmissions (Mills et al., 2020). Hypertension occurs due to the increased flow of blood through the blood vessels which will create restlessness and pain for the patients. However, with the integration of EHR into the healthcare practice this problem can be reduced using alarm strategies for increasing blood pressure or proper monitoring of the levels of blood pressure both by the healthcare providers and the patients. The hypertension rate is higher in older adults and increases more when they reach the age of sixty. It has been found that 60% of older adults suffer from hypertension after the sixties and approximately 75% of women have high blood pressure after the seventies with 65% of men (Oliveros et al., 2020). Noticing all the facts it becomes necessary to understand the reasons for the prevailing health problems regarding hypertension and effectively monitor them for a better outcome for the patient. The American Nurses Association (ANA) also focuses on the safety of the patients and providing high-quality healthcare for the better response of the patients. Using ANA standards the informatics model can be effectively integrated into clinical practice to ensure the efficiency of the treatment provided to the patients for their improved outcomes and effective health problem resolution ​​(Flaubert, 2021). The nursing informatics model of EHR allows nurses to generate early responses regarding deteriorating health situations thus preventing patients from health severities.

Informatics Model

​​The informatics model will be used to effectively monitor the health situation of teh patients regarding the prevailing chronic diseases of hypertension in the EHR system (Lu et al., 2023). This system allows the nursing staff to keep a record of the patients and generate alerts for the effective monitoring of their health situation along with providing quality care and safety for the prevention of healthcare damages. The EHR system helps the nursing staff to effectively monitor the health situation of the patients by generating alerts that will be effective in providing quality care to the patients and reducing the level of blood pressure. The increase in the level of blood pressure should be effectively analyzed by the EHR system for the early intervention of healthcare providers. The effects of the EHR in managing hypertension in patients are given below;
  • The early recognition of the increase in blood pressure for the timely intervention of the healthcare providers.
  • Maintaining the medical history of the patients for accessing it early at the time of need.
  • Maintaining the blood pressure management plans of the patients along with the records of their activity in those plans.
  • The real-time monitoring of the health situation of the patients through the integration of alarm systems in the EHR.
  • Personalized care plans will be provided to the patients for their effectiveness in maintaining the level of blood pressure.
  • The scheduling of the follow-ups to prevent health damage by continuously engaging patients in feedback regarding the care quality.

Literature Review

The Use of the informatics model called EHR helps understand the trend in the healthcare situation regarding hypertension and effectively implements the data collected to improve the responses of the healthcare providers. It has been seen that with the implementation of the EHR the nursing staff can effectively monitor the health situation of the patients by identifying the gaps in the follow-ups and the regular assessment and examination of blood pressure (Coffey et al., 2022). This gap will identify the reason behind the exacerbation of teh health issue of hypertension thus allowing the staff to implement the controlled monitoring system of the EHR that will help the patients in the self-management and effectively determine the lack in the practice that leads to an increase in blood pressure. Once the gaps are identified the implementation and success of the personalized care plans will be ensured. The EHR system allows the nursing staff to effectively conduct follow-ups regarding the health situation of the patients suffering from hypertension for the betterment of the strategies implemented to improve the health response of the patients (Vos et al., 2020). Regular follow-ups help in the identification of the changes in the medications and the health management plan to improve the consistency of the patient with the improvement in the level of blood pressure thus reducing the effects of hypertension. The EHR enhances the ability of the patients to timely consult with their healthcare providers regarding any difficulty in their health situation thus ensuring the timely recognition and response to health problems for the prevention of huge health damages.

Model for Change

​​ADKAR model will be effective in implementing the change in the healthcare organization regarding the integration of EHR in the nursing practice to enhance the quality of the follow-ups and the response of the nurses in ensuring the timely intervention of the healthcare providers to control the levels of exacerbated blood pressure for the improvement in the health condition of the patients (Balluck et al., 2020). The nursing staff should educate individuals regarding the components that can help them in maintaining the level of hypertension along with informing them about the complications related to healthcare problems like heart strokes and many more. This awareness will help the patients to effectively monitor their health situation and implement strategies in their daily routine that will be effective in preventing deteriorating health issues. The nursing staff can improve follow-up by properly guiding the patients regarding the importance of timely follow-ups and regular consultation with the healthcare providers about the change in the health situation for the betterment of their health condition.

The increasing rate of chronic diseases of hypertension among the older age population makes it the need of time for healthcare organizations to implement strategies that are effective in regularly monitoring the level of blood pressure to maintain health without the exacerbation of hypertension (Oliveros et al., 2020). The informatics model of EHR can help the nursing staff ensure regular and timely follow-ups for the improved healthcare response of the patients and decrease the rate of hypertension among individuals. The literature also shows the importance of EHR in improving the healthcare outcomes among patients having the problem of hypertension as it allows the early intervention of healthcare providers for the health improvement of the patient. The ADKAR model of change can ensure the effective integration of EHR in clinical practice.
