After beginning with Seroquel, the patient returns to the clinic in four weeks stating that she has gained weight and the score on Young Mania Rating Scale is decreased from 22 to 18, she reported of sleeping more at bedtime, she also complained of dry mouth and constipated after starting this medication. Considering these symptoms above, the best decision is to discontinue Seroquel and start Geodon 40mg orally twice a day, administer with 500 caloric meals. Ziprasidone (Geodon) is another second-generation antipsychotic medication commonly prescribed for bipolar maintenance (Stahl, 2008). It has a low impact on metabolism that makes it a good choice for bipolar disorder and schizophrenia treatment

(Mandrioli, Protti, Mercolini, 2015). Increasing the dosage or continuing with the same treatment could irritate the patient more with symptoms like dry mouth and constipation. Therefore, discontinuing Seroquel and changing it to Geodon is an appropriate choice to get the best results. What were you hoping to achieve by making this decision? I was hoping to achieve remission in maniac symptoms with no other side effects. In patients with a bipolar maniac, Ziprasidone is considered as first-line treatment (Patel & Keck, 2014).
