In the assessment of the Jordan community in Minnesota, diabetes has been discussed as the major problem affecting a wide range of patients in the healthcare system.  

The technological strategy will be one of the most effective measures that can aid in reducing the number of diabetic cases in the community. The change would require all the nurses and the patient to adapt to the effect of the technological culture that will be adopted to solve the diabetic problem among the population (Fernandez et al., 2019). This will ensure that all patients have an equal chance to interact with the modern technology used to manage diabetes in the community.  

Another benefit of the proposed healthcare changes is the reduced cost of treatment. The diabetic patient spends more funds on managing the disease and visiting the clinics more often for an injection which would be important in managing the disease for a few days. However, with the new technology, patients would have an automated system to control their insulin levels without visiting healthcare organizations. Diabetes is a condition where the body is incapable of producing enough insulin as required or fails to utilize the produced insulin as appropriate (Fernandez et al., 2019). Poorly controlled diabetes is one of the most chronic conditions. The identified population health problem relevant to the practice is poorly controlled diabetes among most patients receiving diabetes care. Currently, patients cater for the cost, such as insulin pumps provided by other organizations. The current finances at the clinic do not allow it to apply better interventions that can significantly reduce the severity of diabetes in the increasing number of patients. 

Potential Barriers to Change in A Community 

The successful implementation plan of this project in the Jordan community will be faced various barriers because it is always a norm that people do not like disruptive change. In most cases, changes involving the community face various barriers that might affect the desired change. One of the potential barriers is inadequate institutional support (Mathieson et al., 2019). The leadership at the clinic may fail to provide the optimum support needed for this project. Strategies such as aligning the missions of the organization and increasing the nurse participating in the project will be adopted to address these potential barriers. 

The other barrier that might be experienced in this process is the need for more interest among nurses operating within community hospitals. Factors such as reduced awareness of the program may contribute to this barrier (Mathieson et al., 2019). The community will need to be involved in the program to ensure that the intervention is implemented as desired. The successful implementation of this project will have a positive patient outcome, especially in addressing issues related to the high number of diabetic cases recorded in the past three years in the community.
